Berenson;Abstract Most patients with advanced multiple myeloma (MM) develop bone lesions during their disease course. Myeloma bone disease can result in potentially debilitating and life threatening skeletal-related events (SREs) such as pathologic fracture, spinal cord compression, the need for palliative radiotherapy (RT) or surgery to bone, and hypercalcemia of malignancy. Bone-targeted therapies that prevent or delay SRE onset may maintain quality of life (QOL) and functional independence in patients with advanced MM. Yet, the risk factors for SREs in this patient population are not fully understood. Exploratory analyses were conducted to identify potential SRE risk factors in patients with bone lesions from MM who received either zoledronic acid or pamidronate every 3 to 4 weeks for up to 24 months in a large, randomized trial. Patients with complete baseline demographics, disease characteristics, and markers of bone metabolism information available were included (n=282). Dichotomous variables included sex, race (white/other), narcotic analgesics (yes/no), Eastern Cooperative Oncology Group performance status (active/impaired), prior SRE (yes/no), and values with a defined upper limit of normal (creatinine, lymphocyte %, hemoglobin, serum glutamic oxaloacetic transaminase, albumin, lactate dehydrogenase [LDH], and calcium). Continuous variables included age, weight, cancer duration, Functional Assessment of Cancer Therapy-General score, Brief Pain Inventory (BPI) score, and bone markers (eg, urinary N-telopeptide of type I collagen [NTX], deoxypyridinoline [DPD]). Paraprotein type was also included. Univariate and multivariate analyses to determine relative risks (RR) for reduced time to first SRE associated with baseline variables using Cox regression models were developed, and those that were not significant at the 5% level were removed by backward elimination to generate a reduced model. In the reduced multivariate model, lower weight (RR=0.94 per 5-kg increase; P=.021), higher BPI scores (RR=1.16 per 1-unit increase; P < .001), race other than white (RR=0.60; P=.028), need for narcotic analgesics (RR=1.61; P=.017), and high levels of NTX (RR=1.68 per 100-nmol/mmol creatinine increase; P=.005) significantly correlated with reduced time to first SRE. Pathologic fracture and RT to bone were the most common SREs; in multivariate models, lower weight and higher BPI scores were associated with increased RRs of both fractures and RT to bone. Race and DPD levels were also significant covariates for fractures, whereas high levels of LDH correlated significantly with need for RT. Because bone resorption marker levels were significant covariates, the correlation between baseline NTX and time to first SRE was assessed. High baseline NTX (≥ 50 nmol/mmol creatinine) was associated with increased risk of shorter time to first SRE: by a significant 67% in the zoledronic acid group (n=210; P=.015) and by a 57% trend in the pamidronate group (n=108; P=.114). Taken together, lower weight and pain parameters (eg, BPI or narcotic analgesics) correlated consistently with skeletal morbidity risks in patients with advanced MM. Treatments that facilitate the restoration of bone homeostasis, as evidenced by bone marker normalization, may reduce the risk of SREs, thus maintaining QOL in this patient population.

Zutshi;A search for direct pair production of supersymmetric top squarks ((t) over tilde (1)) is presented, assuming the (t) over tilde (1) decays into a top quark and the lightest supersymmetric particle, (chi) over tilde (0)(1), and that both top quarks decay to purely hadronic final states. A total of 16 (4) events are observed compared to a predicted standard model background of 13.5(-3.6)(+3.7) (4.4(-1.3)(+1.7)) events in two signal regions based on integral Ldt = 4.7 fb(-1) of pp collision data taken at root s = 7 TeV with the ATLAS detector at the LHC. An exclusion region in the (t) over tilde (1) versus (chi) over tilde (0)(1) mass plane is evaluated: 370 1) 10) similar to 0 GeV while m((t) over tilde1) = 445 GeV is excluded for m((chi) over tilde 10) <= 50 GeV.

Heunks;International audience; Mechanical ventilation may have adverse effects on both the lung and the diaphragm. Injury to the lung is mediated by excessive mechanical stress and strain, whereas the diaphragm develops atrophy as a consequence of low respiratory effort and injury in case of excessive effort. The lung and diaphragm-protective mechanical ventilation approach aims to protect both organs simultaneously whenever possible. This review summarizes practical strategies for achieving lung and diaphragm-protective targets at the bedside, focusing on inspiratory and expiratory ventilator settings, monitoring of inspiratory effort or respiratory drive, management of dyssynchrony, and sedation considerations. A number of potential future adjunctive strategies including extracorporeal CO2 removal, partial neuromuscular blockade, and neuromuscular stimulation are also discussed. While clinical trials to confirm the benefit of these approaches are awaited, clinicians should become familiar with assessing and managing patients' respiratory effort, based on existing physiological principles. To protect the lung and the diaphragm, ventilation and sedation might be applied to avoid excessively weak or very strong respiratory efforts and patient-ventilator dysynchrony.

Major contributors of comp Measurements of the azimuthal anisotropy in lead–lead collisions at sNN−−−√ = 5.02 TeV are presented using a data sample corresponding to 0.49 nb−1 integrated luminosity collected by the ATLAS experiment at the LHC in 2015. The recorded minimum-bias sample is enhanced by triggers for “ultra-central” collisions, providing an opportunity to perform detailed study of flow harmonics in the regime where the initial state is dominated by fluctuations. The anisotropy of the charged-particle azimuthal angle distributions is characterized by the Fourier coefficients, v2–v7, which are measured using the two-particle correlation, scalar-product and event-plane methods. Roche;SCOPE: Insulin resistance (IR) and inflammation are hallmarks of type 2 diabetes (T2D). The nod-like receptor pyrin domain containing-3 (NLRP3) inflammasome is a metabolic sensor activated by saturated fatty acids (SFA) initiating IL-1β inflammation and IR. Interactions between SFA intake and NLRP3-related genetic variants may alter T2D risk factors. METHODS: Meta-analyses of six Cohorts for Heart and Aging Research in Genomic Epidemiology Consortium (n = 19 005) tested interactions between SFA and NLRP3-related single-nucleotide polymorphisms (SNPs) and modulation of fasting insulin, fasting glucose, and homeostasis model assessment of insulin resistance. RESULTS: SFA interacted with rs12143966, wherein each 1% increase in SFA intake increased insulin by 0.0063 IU mL-1 (SE ± 0.002, p = 0.001) per each major (G) allele copy. rs4925663, interacted with SFA (β ± SE = -0.0058 ± 0.002, p = 0.004) to increase insulin by 0.0058 IU mL-1 , per additional copy of the major (C) allele. Both associations are close to the significance threshold (p < 0.0001). rs4925663 causes a missense mutation affecting NLRP3 expression. CONCLUSION: Two NLRP3-related SNPs showed potential interaction with SFA to modulate fasting insulin. Greater dietary SFA intake accentuates T2D risk, which, subject to functional validation, may be further elaborated depending on NLRP3-related genetic variants. Sí

In particular, the chapter proposes intelligent systems that use non-linear optimization as well as symbolic rule-based systems to adjust sound/music, on-stage lighting, and projected visual imagery in terms of movement and color to dynamically reflecting the dancer’s arousal state measured through physiological sensors worn by the dancers. To aesthetically adjust physical lighting around the dancers, dancer’s location will need to be identified. This variable will be identified using pressure sensor mats installed on the floor to track dancers’ movements. Data from these sensors will be passed into a three layered architecture. Layer 1 is composed of a sensor analysis system that analyzes and synthesizes physiological and pressure sensor signals. Layer 2 is composed of intelligent systems that adapt lighting, sound/music, and visual imagery in terms of pacing and colors to portray the dancer’s arousal state. The intelligent physical lighting system dynamically adjusts physical lighting color. The intelligent sound/music system dynamically and unobtrusively adjusts the music/ sound in the space. The virtual projected imagery will be created using a cellular automata system that will be intelligently manipulated in terms of cell colors to reflect the arousal state and position of the dancer. Layer 3 translates the high-level adjustments made by the intelligent systems in layer 2 to appropriate lighting board and audio box commands. Using this architecture we seek to extend the mode of dance expression offering a space that performance artists can use as a creative tool that extends the grammar of dance. In this chapter, we will describe this architecture in detail as well as the equipment, control systems, and Artificial Intelligence algorithms used.

Even though a number of service composition and orchestration techniques have been proposed over the past decade, these do not take into account stakeholders’ intents as well as data, control, and temporal interdependencies between actions microservices can perform. In this paper, we present extensions to goal models with respect to data, logical and temporal dependencies exhibited between tasks and actions among microservices, and we propose a framework based on a graph transformation approach which, when applied to the extended goal models, can yield service invocation plans that achieve the desired requirements and constraints denoted by the specific goal models being considered.

It is a music and visual arts programme designed and implemented with early years teachers of children aged 3–5 years. It explored whether the provision of high-quality arts experiences could enhance children’s emerging literacy and numeracy. A novel ‘artist in(formed) residency’ model was developed within a unique arts education research design. Piloting a tripartite relationship between researchers, artists and early years teachers, this study was situated within a qualitative action research paradigm, with both artists located in both services weekly over a 12-week period. Myeloma bone disease can result in potentially debilitating and life threatening skeletal-related events (SREs) such as pathologic fracture, spinal cord compression, the need for palliative radiotherapy (RT) or surgery to bone, and hypercalcemia of malignancy. Bone-targeted therapies that prevent or delay SRE onset may maintain quality of life (QOL) and functional independence in patients with advanced MM. Yet, the risk factors for SREs in this patient population are not fully understood. Exploratory analyses were conducted to identify potential SRE risk factors in patients with bone lesions from MM who received either zoledronic acid or pamidronate every 3 to 4 weeks for up to 24 months in a large, randomized trial. Patients with complete baseline demographics, disease characteristics, and markers of bone metabolism information available were included (n=282). Dichotomous variables included sex, race (white/other), narcotic analgesics (yes/no), Eastern Cooperative Oncology Group performance status (active/impaired), prior SRE (yes/no), and values with a defined upper limit of normal (creatinine, lymphocyte %, hemoglobin, serum glutamic oxaloacetic transaminase, albumin, lactate dehydrogenase [LDH], and calcium). Continuous variables included age, weight, cancer duration, Functional Assessment of Cancer Therapy-General score, Brief Pain Inventory (BPI) score, and bone markers (eg, urinary N-telopeptide of type I collagen [NTX], deoxypyridinoline [DPD]). Paraprotein type was also included. Univariate and multivariate analyses to determine relative risks (RR) for reduced time to first SRE associated with baseline variables using Cox regression models were developed, and those that were not significant at the 5% level were removed by backward elimination to generate a reduced model. In the reduced multivariate model, lower weight (RR=0.94 per 5-kg increase; P=.021), higher BPI scores (RR=1.16 per 1-unit increase; P < .001), race other than white (RR=0.60; P=.028), need for narcotic analgesics (RR=1.61; P=.017), and high levels of NTX (RR=1.68 per 100-nmol/mmol creatinine increase; P=.005) significantly correlated with reduced time to first SRE. Pathologic fracture and RT to bone were the most common SREs; in multivariate models, lower weight and higher BPI scores were associated with increased RRs of both fractures and RT to bone. Race and DPD levels were also significant covariates for fractures, whereas high levels of LDH correlated significantly with need for RT. Because bone resorption marker levels were significant covariates, the correlation between baseline NTX and time to first SRE was assessed. High baseline NTX (≥ 50 nmol/mmol creatinine) was associated with increased risk of shorter time to first SRE: by a significant 67% in the zoledronic acid group (n=210; P=.015) and by a 57% trend in the pamidronate group (n=108; P=.114). Taken together, lower weight and pain parameters (eg, BPI or narcotic analgesics) correlated consistently with skeletal morbidity risks in patients with advanced MM. Treatments that facilitate the restoration of bone homeostasis, as evidenced by bone marker normalization, may reduce the risk of SREs, thus maintaining QOL in this patient population.

Zutshi;A search for direct pair production of supersymmetric top squarks ((t) over tilde (1)) is presented, assuming the (t) over tilde (1) decays into a top quark and the lightest supersymmetric particle, (chi) over tilde (0)(1), and that both top quarks decay to purely hadronic final states. A total of 16 (4) events are observed compared to a predicted standard model background of 13.5(-3.6)(+3.7) (4.4(-1.3)(+1.7)) events in two signal regions based on integral Ldt = 4.7 fb(-1) of pp collision data taken at root s = 7 TeV with the ATLAS detector at the LHC. An exclusion region in the (t) over tilde (1) versus (chi) over tilde (0)(1) mass plane is evaluated: 370 1) 10) similar to 0 GeV while m((t) over tilde1) = 445 GeV is excluded for m((chi) over tilde 10) <= 50 GeV.

Heunks;International audience; Mechanical ventilation may have adverse effects on both the lung and the diaphragm. Injury to the lung is mediated by excessive mechanical stress and strain, whereas the diaphragm develops atrophy as a consequence of low respiratory effort and injury in case of excessive effort. The lung and diaphragm-protective mechanical ventilation approach aims to protect both organs simultaneously whenever possible. This review summarizes practical strategies for achieving lung and diaphragm-protective targets at the bedside, focusing on inspiratory and expiratory ventilator settings, monitoring of inspiratory effort or respiratory drive, management of dyssynchrony, and sedation considerations. A number of potential future adjunctive strategies including extracorporeal CO2 removal, partial neuromuscular blockade, and neuromuscular stimulation are also discussed. While clinical trials to confirm the benefit of these approaches are awaited, clinicians should become familiar with assessing and managing patients' respiratory effort, based on existing physiological principles. To protect the lung and the diaphragm, ventilation and sedation might be applied to avoid excessively weak or very strong respiratory efforts and patient-ventilator dysynchrony.

Major contributors of comp Measurements of the azimuthal anisotropy in lead–lead collisions at sNN−−−√ = 5.02 TeV are presented using a data sample corresponding to 0.49 nb−1 integrated luminosity collected by the ATLAS experiment at the LHC in 2015. The recorded minimum-bias sample is enhanced by triggers for “ultra-central” collisions, providing an opportunity to perform detailed study of flow harmonics in the regime where the initial state is dominated by fluctuations. Roche;SCOPE: Insulin resistance (IR) and inflammation are hallmarks of type 2 diabetes (T2D). The nod-like receptor pyrin domain containing-3 (NLRP3) inflammasome is a metabolic sensor activated by saturated fatty acids (SFA) initiating IL-1β inflammation and IR. Interactions between SFA intake and NLRP3-related genetic variants may alter T2D risk factors. METHODS: Meta-analyses of six Cohorts for Heart and Aging Research in Genomic Epidemiology Consortium (n = 19 005) tested interactions between SFA and NLRP3-related single-nucleotide polymorphisms (SNPs) and modulation of fasting insulin, fasting glucose, and homeostasis model assessment of insulin resistance. RESULTS: SFA interacted with rs12143966, wherein each 1% increase in SFA intake increased insulin by 0.0063 IU mL-1 (SE ± 0.002, p = 0.001) per each major (G) allele copy. rs4925663, interacted with SFA (β ± SE = -0.0058 ± 0.002, p = 0.004) to increase insulin by 0.0058 IU mL-1 , per additional copy of the major (C) allele. Both associations are close to the significance threshold (p < 0.0001). rs4925663 causes a missense mutation affecting NLRP3 expression. CONCLUSION: Two NLRP3-related SNPs showed potential interaction with SFA to modulate fasting insulin. Greater dietary SFA intake accentuates T2D risk, which, subject to functional validation, may be further elaborated depending on NLRP3-related genetic variants. Sí

In particular, the chapter proposes intelligent systems that use non-linear optimization as well as symbolic rule-based systems to adjust sound/music, on-stage lighting, and projected visual imagery in terms of movement and color to dynamically reflecting the dancer’s arousal state measured through physiological sensors worn by the dancers. To aesthetically adjust physical lighting around the dancers, dancer’s location will need to be identified. This variable will be identified using pressure sensor mats installed on the floor to track dancers’ movements. Data from these sensors will be passed into a three layered architecture. Layer 1 is composed of a sensor analysis system that analyzes and synthesizes physiological and pressure sensor signals. Layer 2 is composed of intelligent systems that adapt lighting, sound/music, and visual imagery in terms of pacing and colors to portray the dancer’s arousal state. The intelligent physical lighting system dynamically adjusts physical lighting color. The intelligent sound/music system dynamically and unobtrusively adjusts the music/ sound in the space. The virtual projected imagery will be created using a cellular automata system that will be intelligently manipulated in terms of cell colors to reflect the arousal state and position of the dancer. Layer 3 translates the high-level adjustments made by the intelligent systems in layer 2 to appropriate lighting board and audio box commands. Using this architecture we seek to extend the mode of dance expression offering a space that performance artists can use as a creative tool that extends the grammar of dance. In this chapter, we will describe this architecture in detail as well as the equipment, control systems, and Artificial Intelligence algorithms used.

Even though a number of service composition and orchestration techniques have been proposed over the past decade, these do not take into account stakeholders’ intents as well as data, control, and temporal interdependencies between actions microservices can perform. In this paper, we present extensions to goal models with respect to data, logical and temporal dependencies exhibited between tasks and actions among microservices, and we propose a framework based on a graph transformation approach which, when applied to the extended goal models, can yield service invocation plans that achieve the desired requirements and constraints denoted by the specific goal models being considered.

It is a music and visual arts programme designed and implemented with early years teachers of children aged 3–5 years. It explored whether the provision of high-quality arts experiences could enhance children’s emerging literacy and numeracy. A novel ‘artist in(formed) residency’ model was developed within a unique arts education research design. Piloting a tripartite relationship between researchers, artists and early years teachers, this study was situated within a qualitative action research paradigm, with both artists located in both services weekly over a 12-week period. The evaluation of the programme indicates changes in pedagogical planning and style in the early years teachers over the period of the artists’ residency.
