    It is unknown whether rituximab increases the risk of second primary malignancies (SPMs) in patients with diffuse large cell B-cell lymphoma (DLBCL). We assessed SPMs in DLBCL patients diagnosed between 1996 and 2014 in comparison with the general Israeli population and dependent on rituximab treatment. Jewish patients had no increased risk for SPMs. Arab-DLBCL females had a higher SPMs rate compared to the general Arab-females population [SIR (95%CI) 1.86 (1.08-2.98)]. Incidence and time to SPMs, in both Jewish and Arab patients, were unaffected by rituximab. Risk for breast and thyroid cancers, in Arab and Jewish females respectively, were higher in the pre-rituximab era [SIR(95%CI) 5.25 (1.41-13.43) and SIR(95%CI) 3.85 (1.41-8.38), respectively]. Age ≥60 years was the only predictor for increased risk of SPM (HR = 2.5,

    A search for direct pair production of supersymmetric top squarks ((t) over tilde (1)) is presented, assuming the (t) over tilde (1) decays into a top quark and the lightest supersymmetric particle, (chi) over tilde (0)(1), and that both top quarks decay to purely hadronic final states. A total of 16 (4) events are observed compared to a predicted standard model background of 13.5(-3.6)(+3.7) (4.4(-1.3)(+1.7)) events in two signal regions based on integral Ldt = 4.7 fb(-1) of pp collision data taken at root s = 7 TeV with the ATLAS detector at the LHC. An exclusion region in the (t) over tilde (1) versus (chi) over tilde (0)(1) mass plane is evaluated: 370 1) 10) similar to 0 GeV while m((t) over tilde1) = 445 GeV is excluded for m((chi) over tilde 10) <= 50 GeV.

    WITH few exceptions, tooth shape among crocodyliform reptiles (Crocodylia of traditional use) is rather uniform1. We report here on the presence of multicuspid molariform teeth in a remarkable new crocodyliform from the Lower Cretaceous of China, which may represent the first known herbivorous member of that group. The overall structure of these teeth is very similar to that of the postcanine teeth of tritylodontid synapsids and represents a particularly striking example of convergent evolution. It indicates back-to-front (proal) motion of the mandible produced by the posterior pterygoid muscle during jaw closing, much as in the extant tuatara, Sphenodon2,3. Certain derived features indicate that the new Chinese crocodyliform is closely related to the Notosuchidae from the Cretaceous of Gondwana4. Its discovery thus casts further doubts on claims5 concerning an endemic Gondwanan tetrapod fauna during the Cretaceous.

    AbstractThe Pax gene eyg is important for Drosophila eye development. eyg expression in the visual system changes dynamically during development. In this study, we found that the transcriptional regulation of eyg can be separated into four distinct temporal phases (E, L1, L2, and L3) and each is regulated by distinct cis-regulatory elements. Utilizing these enhancers for temporal and spatially specific manipulations, we addressed the regulation and function of eyg at different developmental stages. We found that Notch signaling is required and sufficient for eyg expression and this activity is restricted only to the L2 stage. We further showed that the function of eyg in eye development is required only at the second instar larval stage, while its function for head and antenna development can be provided at any time during embryo and larval development. Thus there is a temporal switch of the regulatory mechanism and function of eyg. We propose that eyg expression at L2 is induced and maintained by N signaling, and is turned off at L3 by a negative feedback loop involving the morphogenetic furrow.

The recorded minimum-bias sample is enhanced by triggers for “ultra-central” collisions, providing an opportunity to perform detailed study of flow harmonics in the regime where the initial state is dominated by fluctuations.     Abstract Background/purpose Stem cells (SCs) are characterized by the ability to undergo self-renewal and differentiation into multi-lineage cell types. The low risk to donors, the ease of harvest, and the abundance in the human body of adipose-derived SCs make it an attractive source for adult SCs. Materials and methods Oral adipose tissue was collected by clinical surgery as certified by the Institutional Review Board. SCs were isolated and purified with collagenase containing Dulbecco's modified Eagle's medium (DMEM) and confirmed by immunofluorescence staining with early stage SCs and germinal layer markers. Telomere length was assayed following the procedure provided with the Telo TAGGG kit, and the reverse transcription polymerase chain reaction (RT-PCR) of octamer-binding transcription factor 4 (Oct4) was detected with primers 5′-GTA CTC CTC GGT CCC TTT CC-3′ and 5′-CAA AAA CCC TGG CAC AAA CT-3′. Results SCs derived from human oral adipose tissue (hOASCs) express Oct4 (an early stage stem cell marker) as well as bone morphogenetic protein 4 (BMP4) and nestin (both germ layer markers). Furthermore, the telomere length assay showed that hOASCs possess high-molecular-weight fragments of telomere, an indication of self-renewal capacity. Contrary to previous observations that the differentiation ability of adult SCs is affected by cell age and source of restructure, our results suggested that hOASCs possess pluripotential at the early stage and the ability to continually proliferate without curtailment of the telomere length. Conclusion Human oral adipose tissues can be a beneficial source for isolating pluripotent adult SCs for clinical autologous applications.

    High-fidelity simulation (HFS) is widely used in pediatric emergency medicine (PEM) training and a competent facilitator is vital for effective learning. This survey describes the characteristics, comfort, practices, and need of PEM physicians as HFS facilitators.A descriptive cross-sectional survey was electronically distributed to Pediatric Emergency Research Canada physician members, representing 14 academic pediatric emergency departments nationally.The response rate was 66.6% (92/138); 63% (56/89) of PEM physicians taught HFS. Junior attending physicians (P = 0.011) and those with an education focus (P = 0.005) were more comfortable in using HFS. Sixty-eight percent (38/56) described their facilitator training as formal. Generally, facilitators felt comfortable in running simulations (weighted mean scale, 1.53 [2 = comfortable] on a 5-point rating scale). Facilitators with formal training used verbal confidentiality agreements more frequently (P = 0.008), spent less time running the scenario (P0.05) and spent more time in debriefing (P0.05) than those without formal training. Sixty-three percent (n = 56) of facilitators identified debriefing as the most stressful aspect of HFS. Their main barrier to HFS teaching was lack of protected teaching time (mean scale, 2.02 [2 = barrier]). Seventy-six percent (35/46) of respondents desired online and printable facilitator information. Seventy percent (35/51) thought the ideal time for formal facilitator training was during fellowship.High-fidelity simulation is a widely used educational modality, and more attention must be paid to the needs of the facilitator in order to optimize the educational experience. Standardized facilitator training, focused particularly on effective debriefing techniques, would help improve facilitator comfort with HFS.

    With advances in understanding the role of eosinophils in disease pathogenesis, particularly in the airways, gastrointestinal tract, and skin, targeting eosinophils or the cytokines that lead to their production, activation, and survival has become an increasingly pursued therapeutic approach. Newly developed biologic agents target eosinophils directly, other cells interacting with or activating eosinophils, or cytokines in the type 2 inflammatory pathway with specific antibodies. Current treatment paradigms reserve therapy with biologics for patients refractory to or intolerant of corticosteroids or immunosuppressants. Given accumulating data for safety and efficacy of these biologics, however, there is the question of whether targeted treatments should be used earlier in the treatment algorithm. In this article, we discuss the pros and cons of using biologics as first-line therapy for eosinophilic diseases of the airways, gastrointestinal tract, and skin. We highlight emerging biologic agents and future directions for research, as well as a rationale for the early use of some biologics to prevent tissue damage, disease progression, and organ dysfunction in selected conditions.

    It is unknown whether rituximab increases the risk of second primary malignancies (SPMs) in patients with diffuse large cell B-cell lymphoma (DLBCL). We assessed SPMs in DLBCL patients diagnosed between 1996 and 2014 in comparison with the general Israeli population and dependent on rituximab treatment. Jewish patients had no increased risk for SPMs. Arab-DLBCL females had a higher SPMs rate compared to the general Arab-females population [SIR (95%CI) 1.86 (1.08-2.98)]. Incidence and time to SPMs, in both Jewish and Arab patients, were unaffected by rituximab. Risk for breast and thyroid cancers, in Arab and Jewish females respectively, were higher in the pre-rituximab era [SIR(95%CI) 5.25 (1.41-13.43) and SIR(95%CI) 3.85 (1.41-8.38), respectively].     A search for direct pair production of supersymmetric top squarks ((t) over tilde (1)) is presented, assuming the (t) over tilde (1) decays into a top quark and the lightest supersymmetric particle, (chi) over tilde (0)(1), and that both top quarks decay to purely hadronic final states. A total of 16 (4) events are observed compared to a predicted standard model background of 13.5(-3.6)(+3.7) (4.4(-1.3)(+1.7)) events in two signal regions based on integral Ldt = 4.7 fb(-1) of pp collision data taken at root s = 7 TeV with the ATLAS detector at the LHC. An exclusion region in the (t) over tilde (1) versus (chi) over tilde (0)(1) mass plane is evaluated: 370 1) 10) similar to 0 GeV while m((t) over tilde1) = 445 GeV is excluded for m((chi) over tilde 10) <= 50 GeV.

    WITH few exceptions, tooth shape among crocodyliform reptiles (Crocodylia of traditional use) is rather uniform1. We report here on the presence of multicuspid molariform teeth in a remarkable new crocodyliform from the Lower Cretaceous of China, which may represent the first known herbivorous member of that group. The overall structure of these teeth is very similar to that of the postcanine teeth of tritylodontid synapsids and represents a particularly striking example of convergent evolution. It indicates back-to-front (proal) motion of the mandible produced by the posterior pterygoid muscle during jaw closing, much as in the extant tuatara, Sphenodon2,3. Certain derived features indicate that the new Chinese crocodyliform is closely related to the Notosuchidae from the Cretaceous of Gondwana4. Its discovery thus casts further doubts on claims5 concerning an endemic Gondwanan tetrapod fauna during the Cretaceous.

    AbstractThe Pax gene eyg is important for Drosophila eye development. eyg expression in the visual system changes dynamically during development. In this study, we found that the transcriptional regulation of eyg can be separated into four distinct temporal phases (E, L1, L2, and L3) and each is regulated by distinct cis-regulatory elements. Utilizing these enhancers for temporal and spatially specific manipulations, we addressed the regulation and function of eyg at different developmental stages. We found that Notch signaling is required and sufficient for eyg expression and this activity is restricted only to the L2 stage. We further showed that the function of eyg in eye development is required only at the second instar larval stage, while its function for head and antenna development can be provided at any time during embryo and larval development. Thus there is a temporal switch of the regulatory mechanism and function of eyg. We propose that eyg expression at L2 is induced and maintained by N signaling, and is turned off at L3 by a negative feedback loop involving the morphogenetic furrow.

    Abstract Background/purpose Stem cells (SCs) are characterized by the ability to undergo self-renewal and differentiation into multi-lineage cell types. The low risk to donors, the ease of harvest, and the abundance in the human body of adipose-derived SCs make it an attractive source for adult SCs. Materials and methods Oral adipose tissue was collected by clinical surgery as certified by the Institutional Review Board. SCs were isolated and purified with collagenase containing Dulbecco's modified Eagle's medium (DMEM) and confirmed by immunofluorescence staining with early stage SCs and germinal layer markers. Telomere length was assayed following the procedure provided with the Telo TAGGG kit, and the reverse transcription polymerase chain reaction (RT-PCR) of octamer-binding transcription factor 4 (Oct4) was detected with primers 5′-GTA CTC CTC GGT CCC TTT CC-3′ and 5′-CAA AAA CCC TGG CAC AAA CT-3′. Results SCs derived from human oral adipose tissue (hOASCs) express Oct4 (an early stage stem cell marker) as well as bone morphogenetic protein 4 (BMP4) and nestin (both germ layer markers). Furthermore, the telomere length assay showed that hOASCs possess high-molecular-weight fragments of telomere, an indication of self-renewal capacity. Contrary to previous observations that the differentiation ability of adult SCs is affected by cell age and source of restructure, our results suggested that hOASCs possess pluripotential at the early stage and the ability to continually proliferate without curtailment of the telomere length. Conclusion Human oral adipose tissues can be a beneficial source for isolating pluripotent adult SCs for clinical autologous applications.

    High-fidelity simulation (HFS) is widely used in pediatric emergency medicine (PEM) training and a competent facilitator is vital for effective learning. This survey describes the characteristics, comfort, practices, and need of PEM physicians as HFS facilitators.A descriptive cross-sectional survey was electronically distributed to Pediatric Emergency Research Canada physician members, representing 14 academic pediatric emergency departments nationally.The response rate was 66.6% (92/138); 63% (56/89) of PEM physicians taught HFS. Junior attending physicians (P = 0.011) and those with an education focus (P = 0.005) were more comfortable in using HFS. Sixty-eight percent (38/56) described their facilitator training as formal. Generally, facilitators felt comfortable in running simulations (weighted mean scale, 1.53 [2 = comfortable] on a 5-point rating scale). Facilitators with formal training used verbal confidentiality agreements more frequently (P = 0.008), spent less time running the scenario (P0.05) and spent more time in debriefing (P0.05) than those without formal training. Sixty-three percent (n = 56) of facilitators identified debriefing as the most stressful aspect of HFS. Their main barrier to HFS teaching was lack of protected teaching time (mean scale, 2.02 [2 = barrier]). Seventy-six percent (35/46) of respondents desired online and printable facilitator information. Seventy percent (35/51) thought the ideal time for formal facilitator training was during fellowship.High-fidelity simulation is a widely used educational modality, and more attention must be paid to the needs of the facilitator in order to optimize the educational experience. Standardized facilitator training, focused particularly on effective debriefing techniques, would help improve facilitator comfort with HFS.

    With advances in understanding the role of eosinophils in disease pathogenesis, particularly in the airways, gastrointestinal tract, and skin, targeting eosinophils or the cytokines that lead to their production, activation, and survival has become an increasingly pursued therapeutic approach. Newly developed biologic agents target eosinophils directly, other cells interacting with or activating eosinophils, or cytokines in the type 2 inflammatory pathway with specific antibodies. Current treatment paradigms reserve therapy with biologics for patients refractory to or intolerant of corticosteroids or immunosuppressants. Given accumulating data for safety and efficacy of these biologics, however, there is the question of whether targeted treatments should be used earlier in the treatment algorithm. In this article, we discuss the pros and cons of using biologics as first-line therapy for eosinophilic diseases of the airways, gastrointestinal tract, and skin. We highlight emerging biologic agents and future directions for research, as well as a rationale for the early use of some biologics to prevent tissue damage, disease progression, and organ dysfunction in selected conditions.
