    Proteus syndrome is a complex disorder comprising malformations and overgrowth of multiple tissues. The disorder is highly variable and appears to affect patients in a mosaic manner. This intrinsic variability has led to diagnostic confusion associated with a dearth of longitudinal data on the natural history of Proteus syndrome. To clarify some of these issues, a workshop on Proteus syndrome was held in March 1998 at the National Institutes of Health, and participants developed recommendations for diagnostic criteria, differential diagnosis, and guidelines for the evaluation of patients. This is a review of those recommendations.

    Epstein-Barr virus (EBV) is transmitted by saliva and is a major cause of cancer, particularly in people living with HIV/AIDS. Here, we describe the frequency and quantity of EBV detection in the saliva of Ugandan adults with and without HIV-1 infection and use these data to develop a novel mathematical model of EBV infection in the tonsils. Eligible cohort participants were not taking antiviral medications, and those with HIV-1 infection had a CD4 count >200 cells/mm3. Over a 4-week period, participants provided daily oral swabs that we analysed for the presence and quantity of EBV. Compared with HIV-1 uninfected participants, HIV-1 coinfected participants had an increased risk of EBV detection in their saliva (IRR = 1.27, 95% CI = 1.10–1.47) and higher viral loads in positive samples. We used these data to develop a stochastic, mechanistic mathematical model that describes the dynamics of EBV, infected cells, and immune response within the tonsillar epithelium to analyse potential factors that may cause EBV infection to be more severe in HIV-1 coinfected participants. The model, fit using Approximate Bayesian Computation, showed high fidelity to daily oral shedding data and matched key summary statistics. When evaluating how model parameters differed among participants with and without HIV-1 coinfection, results suggest HIV-1 coinfected individuals have higher rates of B cell reactivation, which can seed new infection in the tonsils and lower rates of an EBV-specific immune response. Subsequently, both these traits may explain higher and more frequent EBV detection in the saliva of HIV-1 coinfected individuals. Author summary Epstein-Barr virus (EBV) is a ubiquitous infection worldwide associated with the development of several kinds of cancer, including B cell lymphoma and nasopharyngeal carcinoma. Rates of EBV replication and disease are higher in individuals who are coinfected with HIV-1. HIV-1 infection is associated with increased B cell activation as well as immunodeficiency resulting from loss of T cells; however, whether these factors contribute to higher rates of EBV replication during coinfection, and by how much, has remained unknown. We analysed oral EBV shedding data from a cohort of Ugandan adults taken at multiple time points and found that participants coinfected with HIV-1 maintained higher quantities of EBV in their saliva. To better understand this finding, we developed a mathematical model to describe the dynamics of EBV infection within the tonsils. By rigorously matching our model to our participant data, we determined that both high rates of infected B cell activation and worse cellular immune control of EBV may cause higher EBV loads in saliva during HIV-1 coinfection. These results help explain the impact of HIV-1 on EBV and suggest potential therapeutic targets to prevent EBV-related malignancy in people who are coinfected with HIV-1.

    A search for direct pair production of supersymmetric top squarks ((t) over tilde (1)) is presented, assuming the (t) over tilde (1) decays into a top quark and the lightest supersymmetric particle, (chi) over tilde (0)(1), and that both top quarks decay to purely hadronic final states. A total of 16 (4) events are observed compared to a predicted standard model background of 13.5(-3.6)(+3.7) (4.4(-1.3)(+1.7)) events in two signal regions based on integral Ldt = 4.7 fb(-1) of pp collision data taken at root s = 7 TeV with the ATLAS detector at the LHC. An exclusion region in the (t) over tilde (1) versus (chi) over tilde (0)(1) mass plane is evaluated: 370 1) 10) similar to 0 GeV while m((t) over tilde1) = 445 GeV is excluded for m((chi) over tilde 10) <= 50 GeV.

    With the rising frequency of pathogen spillover worldwide, wildlife disease dynamics have received increased attention. There are many possible pathway a pathogen can invade and spread through a host population, and the assumed transmission model used to capture disease propagation can influence predictions of pathogen net reproductive success (R0), determining the outbreak dynamics. We synthesize a comprehensive overview of these models and overarching implications, using bovine Tuberculosis (Mycobacterium bovis) as a case study. We unify sub-models from the disease ecology literature and clarify the biological motivation behind these models and resulting ecological dynamics. We warn readers of pitfalls regarding the relative orders of the transmission parameters and reiterate that the contact rate determines the transmission model and thus defines key dynamical properties of an outbreak. Transmission in wildlife is linked to ecosystem and human health, and host community structure can mediate pathogen spread. We link these models with disease-biodiversity theories, by considering the role of host diversity in disease transmission, contributing to the debate on the effect of biodiversity and on disease outbreak potential. We decompose the various mechanisms of transmission in a stepwise process, and provide the reader a guide for modelling pathogens in both single-host and multi-host systems.

    Background. Given the elevated risk of tuberculosis (TB), including drug-resistant disease, experienced by health workers in South Africa (SA), effective workers’ compensation for occupational TB is a legal right and an essential social benefit. Objectives. To investigate the experience of the workers’ compensation system among health workers who suffered from TB while working in public service facilities in Western Cape Province, SA. Methods. In this case series with a qualitative component, 300 claims for occupational TB in health workers were sampled from the provincial health department database of claims submitted. Claim status for each case was ascertained. An attempt was made to contact each health worker for a telephonic interview consisting of both closed- and open-ended (qualitative) questions. Fifty-one interviews were completed. Results. In nearly half of the cases, there was no record of claim status on the state Compensation Fund website. Of the 51 interviewees, only one had received all the compensation benefits for their particular claim circumstances. Health workers’ experience of having their cases reported for compensation purposes was marred by perception of poor communication and administration. The experience of contracting TB was further characterised by surprise, perceptions of stigma, financial burden and ongoing ill-health. Conclusions. Affected health workers’ experience of the workers’ compensation system was mostly negative, adding to the burden of being ill with TB. Education of management and clinicians, improvement in communication, and timeous and regular checking of claim status and of payment of applicable compensation are required at the provincial level. Dedicated facility-based occupational health units are needed, with a staff complement of knowledgeable persons trusted by their colleagues. However, the effectiveness of the system is ultimately dependent on the ability of the Compensation Fund to register and display claims timeously and administer compensation expeditiously.

    AbstractBackgroundThe focus of many data collection efforts centers on creation of more granular data. The assumption is that more complex data are better able to predict outcomes. We hypothesized that data are often needlessly complex. We sought to demonstrate this concept by examination of the American Society of Anesthesiologists (ASA) scoring system.MethodsFirst, we created every possible consecutive two, three and four category combinations of the current five category ASA score. This resulted in 14 combinations of simplified ASA. We compared the predictive ability of these simplified scores for postoperative outcomes for 2.3 million patients in the NSQIP database. Individual model performance was assessed by comparing receiver operator characteristic (ROC) curves for each model with the standard ASA.ResultsTwo of our 4‐category models and one of our 3‐category models had ability to predict all outcomes equivalent to standard ASA. These results held for all outcomes and on all subgroups tested. The performance of the three best performing simplified ASA scores were also equivalent to the standard ASA score in the univariate analysis and when included in a multivariate model.ConclusionsIt is assumed that the most granular data and use of the largest number of variables for risk‐adjusted predictions will increase accuracy. This complexity is often at the expense of utility. Using the single best predictor in surgical outcomes research, we have shown this is not the case. In this example, we demonstrate that one can simplify ASA into a 3‐category variable without losing any ability to predict outcomes.

    The entire southern sky (Declination, $\delta $ 4 Jy) in the extragalactic catalogue (Galactic latitude, $|b| >$ 10 deg). We refer to these 1,863 sources as the GLEAM 4-Jy (G4Jy) Sample, and use radio images (of $\leq$ 45-arcsec resolution), and multi-wavelength information, to assess their morphology and identify the galaxy that is hosting the radio emission (where appropriate). Details of how to access all of the overlays used for this work are available at https://github.com/svw26/G4Jy. Alongside this we conduct further checks against the literature, which we document in this paper for individual sources. Whilst the vast majority of the G4Jy Sample are active galactic nuclei with powerful radio-jets, we highlight that it also contains a nebula, two nearby, star-forming galaxies, a cluster relic, and a cluster halo. There are also three extended sources for which we are unable to infer the mechanism that gives rise to the low-frequency emission. In the G4Jy catalogue we provide mid-infrared identifications for 86% of the sources, and flag the remainder as: having an uncertain identification (129 sources), having a faint/uncharacterised mid-infrared host (126 sources), or it being inappropriate to specify a host (2 sources). For the subset of 129 sources, there is ambiguity concerning candidate host-galaxies, and this includes four sources (B0424$-$728, B0703$-$451, 3C 198, and 3C 403.1) where we question the existing identification.     Abstract Discrepancies between desired ecosystem services (the types, amounts and qualities of services a person would ideally like to benefit from) and the actual provision of ecosystem services (services actually obtained) reflect people’s inability to receive the benefits they would like from nature, and can lead to conflict. We developed a novel process that combined stakeholder analysis, a survey, and a workshop to generate information about discrepancies between desired and provided ecosystem services, and facilitate community-based development of ideas for actions to reduce these discrepancies. We applied this process in a region of Quebec, Canada, where our results show how community members benefit from numerous ecosystem services on diverse property types, but they benefit unequally. Using our process, community members developed ideas for actions to reduce most identified discrepancies. Actions centered around the following themes: developing infrastructure; controlling access; gaining status; encouraging involvement; adjusting natural infrastructure; increasing services; and, information transmission. Findings related to our process also emerged: different knowledge types complement each other to create a broader picture of how people benefit from nature; assessing satisfaction helps uncover nuances associated with discrepancies; knowledge co-creation helps develop ideas for actions to reduce discrepancies; and, considering different perspectives can help avoid trade-offs.

    Proteus syndrome is a complex disorder comprising malformations and overgrowth of multiple tissues. The disorder is highly variable and appears to affect patients in a mosaic manner. This intrinsic variability has led to diagnostic confusion associated with a dearth of longitudinal data on the natural history of Proteus syndrome. To clarify some of these issues, a workshop on Proteus syndrome was held in March 1998 at the National Institutes of Health, and participants developed recommendations for diagnostic criteria, differential diagnosis, and guidelines for the evaluation of patients. This is a review of those recommendations.

    Epstein-Barr virus (EBV) is transmitted by saliva and is a major cause of cancer, particularly in people living with HIV/AIDS. Here, we describe the frequency and quantity of EBV detection in the saliva of Ugandan adults with and without HIV-1 infection and use these data to develop a novel mathematical model of EBV infection in the tonsils. Eligible cohort participants were not taking antiviral medications, and those with HIV-1 infection had a CD4 count >200 cells/mm3. Over a 4-week period, participants provided daily oral swabs that we analysed for the presence and quantity of EBV. Compared with HIV-1 uninfected participants, HIV-1 coinfected participants had an increased risk of EBV detection in their saliva (IRR = 1.27, 95% CI = 1.10–1.47) and higher viral loads in positive samples. We used these data to develop a stochastic, mechanistic mathematical model that describes the dynamics of EBV, infected cells, and immune response within the tonsillar epithelium to analyse potential factors that may cause EBV infection to be more severe in HIV-1 coinfected participants. The model, fit using Approximate Bayesian Computation, showed high fidelity to daily oral shedding data and matched key summary statistics. When evaluating how model parameters differed among participants with and without HIV-1 coinfection, results suggest HIV-1 coinfected individuals have higher rates of B cell reactivation, which can seed new infection in the tonsils and lower rates of an EBV-specific immune response. Subsequently, both these traits may explain higher and more frequent EBV detection in the saliva of HIV-1 coinfected individuals. Author summary Epstein-Barr virus (EBV) is a ubiquitous infection worldwide associated with the development of several kinds of cancer, including B cell lymphoma and nasopharyngeal carcinoma. Rates of EBV replication and disease are higher in individuals who are coinfected with HIV-1. HIV-1 infection is associated with increased B cell activation as well as immunodeficiency resulting from loss of T cells; however, whether these factors contribute to higher rates of EBV replication during coinfection, and by how much, has remained unknown. We analysed oral EBV shedding data from a cohort of Ugandan adults taken at multiple time points and found that participants coinfected with HIV-1 maintained higher quantities of EBV in their saliva. To better understand this finding, we developed a mathematical model to describe the dynamics of EBV infection within the tonsils. By rigorously matching our model to our participant data, we determined that both high rates of infected B cell activation and worse cellular immune control of EBV may cause higher EBV loads in saliva during HIV-1 coinfection. These results help explain the impact of HIV-1 on EBV and suggest potential therapeutic targets to prevent EBV-related malignancy in people who are coinfected with HIV-1.

    A search for direct pair production of supersymmetric top squarks ((t) over tilde (1)) is presented, assuming the (t) over tilde (1) decays into a top quark and the lightest supersymmetric particle, (chi) over tilde (0)(1), and that both top quarks decay to purely hadronic final states. A total of 16 (4) events are observed compared to a predicted standard model background of 13.5(-3.6)(+3.7) (4.4(-1.3)(+1.7)) events in two signal regions based on integral Ldt = 4.7 fb(-1) of pp collision data taken at root s = 7 TeV with the ATLAS detector at the LHC. An exclusion region in the (t) over tilde (1) versus (chi) over tilde (0)(1) mass plane is evaluated: 370 1) 10) similar to 0 GeV while m((t) over tilde1) = 445 GeV is excluded for m((chi) over tilde 10) <= 50 GeV.

    With the rising frequency of pathogen spillover worldwide, wildlife disease dynamics have received increased attention. There are many possible pathway a pathogen can invade and spread through a host population, and the assumed transmission model used to capture disease propagation can influence predictions of pathogen net reproductive success (R0), determining the outbreak dynamics. We synthesize a comprehensive overview of these models and overarching implications, using bovine Tuberculosis (Mycobacterium bovis) as a case study. We unify sub-models from the disease ecology literature and clarify the biological motivation behind these models and resulting ecological dynamics. We warn readers of pitfalls regarding the relative orders of the transmission parameters and reiterate that the contact rate determines the transmission model and thus defines key dynamical properties of an outbreak. Transmission in wildlife is linked to ecosystem and human health, and host community structure can mediate pathogen spread. We link these models with disease-biodiversity theories, by considering the role of host diversity in disease transmission, contributing to the debate on the effect of biodiversity and on disease outbreak potential. We decompose the various mechanisms of transmission in a stepwise process, and provide the reader a guide for modelling pathogens in both single-host and multi-host systems.

    Background. Given the elevated risk of tuberculosis (TB), including drug-resistant disease, experienced by health workers in South Africa (SA), effective workers’ compensation for occupational TB is a legal right and an essential social benefit. Objectives. To investigate the experience of the workers’ compensation system among health workers who suffered from TB while working in public service facilities in Western Cape Province, SA. Methods. In this case series with a qualitative component, 300 claims for occupational TB in health workers were sampled from the provincial health department database of claims submitted. Claim status for each case was ascertained. An attempt was made to contact each health worker for a telephonic interview consisting of both closed- and open-ended (qualitative) questions. Fifty-one interviews were completed. Results. In nearly half of the cases, there was no record of claim status on the state Compensation Fund website. Of the 51 interviewees, only one had received all the compensation benefits for their particular claim circumstances. Health workers’ experience of having their cases reported for compensation purposes was marred by perception of poor communication and administration. The experience of contracting TB was further characterised by surprise, perceptions of stigma, financial burden and ongoing ill-health. Conclusions. Affected health workers’ experience of the workers’ compensation system was mostly negative, adding to the burden of being ill with TB. Education of management and clinicians, improvement in communication, and timeous and regular checking of claim status and of payment of applicable compensation are required at the provincial level. Dedicated facility-based occupational health units are needed, with a staff complement of knowledgeable persons trusted by their colleagues. However, the effectiveness of the system is ultimately dependent on the ability of the Compensation Fund to register and display claims timeously and administer compensation expeditiously.

    AbstractBackgroundThe focus of many data collection efforts centers on creation of more granular data. The assumption is that more complex data are better able to predict outcomes. We hypothesized that data are often needlessly complex. We sought to demonstrate this concept by examination of the American Society of Anesthesiologists (ASA) scoring system.MethodsFirst, we created every possible consecutive two, three and four category combinations of the current five category ASA score. This resulted in 14 combinations of simplified ASA. We compared the predictive ability of these simplified scores for postoperative outcomes for 2.3 million patients in the NSQIP database. Individual model performance was assessed by comparing receiver operator characteristic (ROC) curves for each model with the standard ASA.ResultsTwo of our 4‐category models and one of our 3‐category models had ability to predict all outcomes equivalent to standard ASA. These results held for all outcomes and on all subgroups tested. The performance of the three best performing simplified ASA scores were also equivalent to the standard ASA score in the univariate analysis and when included in a multivariate model.ConclusionsIt is assumed that the most granular data and use of the largest number of variables for risk‐adjusted predictions will increase accuracy. This complexity is often at the expense of utility. Using the single best predictor in surgical outcomes research, we have shown this is not the case. In this example, we demonstrate that one can simplify ASA into a 3‐category variable without losing any ability to predict outcomes.

    The entire southern sky (Declination, $\delta $ 4 Jy) in the extragalactic catalogue (Galactic latitude, $|b| >$ 10 deg). We refer to these 1,863 sources as the GLEAM 4-Jy (G4Jy) Sample, and use radio images (of $\leq$ 45-arcsec resolution), and multi-wavelength information, to assess their morphology and identify the galaxy that is hosting the radio emission (where appropriate). Details of how to access all of the overlays used for this work are available at https://github.com/svw26/G4Jy. Alongside this we conduct further checks against the literature, which we document in this paper for individual sources. Whilst the vast majority of the G4Jy Sample are active galactic nuclei with powerful radio-jets, we highlight that it also contains a nebula, two nearby, star-forming galaxies, a cluster relic, and a cluster halo. There are also three extended sources for which we are unable to infer the mechanism that gives rise to the low-frequency emission. In the G4Jy catalogue we provide mid-infrared identifications for 86% of the sources, and flag the remainder as: having an uncertain identification (129 sources), having a faint/uncharacterised mid-infrared host (126 sources), or it being inappropriate to specify a host (2 sources). For the subset of 129 sources, there is ambiguity concerning candidate host-galaxies, and this includes four sources (B0424$-$728, B0703$-$451, 3C 198, and 3C 403.1) where we question the existing identification.     Abstract Discrepancies between desired ecosystem services (the types, amounts and qualities of services a person would ideally like to benefit from) and the actual provision of ecosystem services (services actually obtained) reflect people’s inability to receive the benefits they would like from nature, and can lead to conflict. We developed a novel process that combined stakeholder analysis, a survey, and a workshop to generate information about discrepancies between desired and provided ecosystem services, and facilitate community-based development of ideas for actions to reduce these discrepancies. We applied this process in a region of Quebec, Canada, where our results show how community members benefit from numerous ecosystem services on diverse property types, but they benefit unequally. Using our process, community members developed ideas for actions to reduce most identified discrepancies. Actions centered around the following themes: developing infrastructure; controlling access; gaining status; encouraging involvement; adjusting natural infrastructure; increasing services; and, information transmission. Findings related to our process also emerged: different knowledge types complement each other to create a broader picture of how people benefit from nature; assessing satisfaction helps uncover nuances associated with discrepancies; knowledge co-creation helps develop ideas for actions to reduce discrepancies; and, considering different perspectives can help avoid trade-offs.
